Applying this patch put a big smile on my face - it does indeed fix the main offset calculation

However, running my test suite, I now see very sporadic errors which makes me think there's
still a race condition lurking somewhere. Running my pagination tests suite 100 times results
in about 82k requests to CouchDB and 8k test assertions. I had 8 assertions fail over the
100 test runs. I'm fairly sure the issue doesn't lie with my test suite or lib.

I'll have a go at creating a test that reproduces the failure.
