XmlTokenSource.save() does not use specified character encoding

It is by design that saving to a Writer does not save out the xml declaration 
(which contains the character encoding).  Because a Writer is character based, 
no encoding takes place.  However, when you save out to a byte based output 
format (OutputStream, for example) the characters get encoded and I save out 
the xml decl and character encoding to express this.
