Short answer: "{urn:PFE-FOO}document" and "document" are different names. Your 
stylesheet only selects the latter (i.e. the one with no namespace).

Please use the correct mailing list for asking questions. See 
http://xml.apache.org/mail.html#xalan-j-users (and also 
http://www.mulberrytech.com/xsl/xsl-list/ for general discussion on XSLT).

The bug tracking system is only meant for reporting bugs and even if this was a 
valid bug report this isn't the right bug database or component. Xalan uses 
JIRA (http://issues.apache.org/jira) for bug tracking.
