I don't think there's any open question. To quote Roy:

"No.  Add an ASF header if we manage the code because it satisfies the BSD
conditions and satisfies our conditions, regardless of the existence of any other licensing
within a file. There is no requirement to ADD a BSD header, anywhere."

That's what I'm proposing -- to follow Roy's advice and DO add ASF headers to all of the files
that came from original BSD license and we do it in bulk. This covers both the case where
we later modified the file as part of ASF contributions and the case where no such modifications
were made.

In addition to this: https://github.com/apache/incubator-madlib/blob/master/LICENSE#L311 I
believe we will be doing the right thing by ASF.
