Sebb suggested and then I revised slightly to satisfy Noah's concern about the version number:

   This file is a component of Apache Foo which is licensed to you under Apache License 2.0.
   Apache Foo is Copyright (C) 2013 The Apache Software Foundation.

I believe this satisfies Dennis' use case also.

Since Apache License 2.0 section 4(c) refers also to trademark notices, you might consider
adding to the header:

    "Apache" and "Apache Foo" are trademarks of The Apache Software Foundation.

These three (3) sentences are entirely sufficient as a full header or short header. I never
understood why we added all that other wording to our longer header.

Does it apply to the previous sentence or the subsequent contents? How about:

License 2.0.
