Subject software grant: best practices

the software grant form is the way that the ASF accepts donations of
code developed outside. i know that this has been done many times but i
couldn't find any detailed documentation on best practise. i'll probably
write something up for the foundation website once i'm clear what the
best practise actually is...

i've done some digging and discovered that a typical method of
completing this form is to attach (as the exhibit) an md5 sum created
using a known version of a software application from the donated source
tarball. the tarball is then committed. is this sufficient?

any other known pitfalls?
