Cool, the debug message helps me. I tried it early in the morning and find the root cause:
WARNING: cache unable to open '/cache/trafficserver/cache.db': Permission denied

You can enable debugging tags in records.config and set the tag string to "cache" (no quotes).You
should then get a lot of messages about the request and why or why not ATS cached it.

curl --head http://url/ will show the response headers
