I'm not sure what you mean by "clean installation", but let's see if my steps qualify as a
clean install:
1.) Stop TomEE
2.) rm -rf tomee-plus-1.7.0/webapps/<exploded_war_directory>
3.) rm -rf tomee-plus-1.7.0/webapps/<war_file>
4.) rm -rf tomee-plus-1.7.0/work/Catalina/localhost
5.) rm -rf tomee-plus-1.7.0/work/Catalina/<folder as result of Host alias>
6.) ...and just to remove any extra clutter, rm tomee-plus-1.7.0/logs/*
7.) copy fresh .war to the webapps directory
8.) Start TomEE.

These are the steps that I take.

O.K.  So, I've attempted {appId}/{ejbJarId}/{ejbName} before after Googling the error and
coming across only about 2 unique posts, both of which you chimed in and suggested that workaround.
 I've been confused on how to use that line of config.  I assume those are placeholders. 
I'm using just a war file, so there's no ejb jar file in use.  This may seem like a dumb question,
but where do I find the appId?  It seems like the id specified in web.xml.

Also, do I need to specify a line of config per EJB?

If my app id is "os" and one of my EJB's is named (with no specified id
annotation), then would the config look like the following?
