This just really doesn't seem like a difficult thing to do at all.

All you're doing is re-writing /item_123.html to
/getItemPage?itemname=123 (whether it includes "extra" /s or not). This
should be trivially done with urlrewrite (http://tuckey.org/urlrewrite/).
