That's it, there's nothing after these lines when I access my
application. No errors, just the debug lines. I've checked
isapi_redirect.dll has execute and write permissions.
I doubt on the below mention log entry, I think instead of localhost it
should point towards my application server machine.

My W3SVC1 log looks like this:

And what happens then?
It is obvious from the log that the uri mapping stage
was OK. Do you have any errors in the log?
If not, perhaps you don't have executable permissions
for the isapi_redirect.dll
