I got it.  It was the invoker servlet mapping.  The invoker servlet was
uncommented by default but the mapping was not.  So for future reference I
uncommented the following lines in my $CATALINE_HOME/conf/web.xml file:

<servlet-mapping>
    <servlet-name>invoker</servlet-name>
   <url-pattern>/servlet/*</url-pattern>
</servlet-mapping>
