You were right.  I upgraded to 4.0.3 and everything seems to be working
fine.  I was staying at 4.0.1 because of a bug in IE and cache-ing PDFs.  I
guess I'll have to fix that problem instead of not upgrading.

I think this web.xml is a 4.0.1 issue..... I like improvements in
4.0.2-4.0.4b2, again, this goes back to all the confusion on versions /
builds, etc.... but I see no reason to run an older version really...
