I do have some progress to report.

The issues you are seeing with test case 2 are due to the context.reload()
calling context.stop() followed by context.start()

context.start() unregisters and re-registers the context with JMX. Whilst the
context is unregistered it does not appear in the mapper so the request is
mapped to the ROOT webapp and a 404 results. I am currently looking for the
least ugly hack around this issue.

At first glance test case 3 is a separate issue which I will look at once I
have test case 2 working properly.
