I hope Im not jumping the gun on this, but it appears this may have been our 
fault.

We have a servlet that acts as a poller updating datamodels as needed.  The 
datamodels are created with the request object and in one case the data model 
was caching it.  Upon update of this particular datamodel getParameter was 
called on the cached request object (setting the flag to true in 
parseParameters) meanwhile on another thread TestServlet was being called with 
this same request object.
