Serg Maslyukov was partially right, thanks.  Apparently, the values passed must 
be in a valid URL encoded format.  The "%" caused the problem, which resulted 
in niether params getting set.  The "?" does not seem to cause any problems.  
The following code works:

<jsp:include page="/NASTCHeader.jsp">
    <jsp:param name="pageTitle" value="Create Invoice?" />
    <jsp:param name="pageWidth" value="100%25" />
</jsp:include>
