Add default exception handler attribute to <global-exceptions> entity

Summary: Add default exception handler attribute to <global-
                    exceptions> entity
           Product: Struts
           Version: 1.1 Beta 2
          Platform: Other
        OS/Version: Other
            Status: NEW
          Severity: Enhancement
          Priority: Other
         Component: Controller
        AssignedTo: struts-dev@jakarta.apache.org
        ReportedBy: beskrowni@alumni.cmu.edu

currently, if you roll your own ExceptionHandler, you need to specify it in
the "handler" attribute of every exception you configure in the struts-config.
it would be nice if you could specify a default (similar to the "type"
attribute in the "actions" tag).
