	* README (Index): Update section names and numbers.
	(VisualStudio Directory Structure): Add new section 
	describing expected directory structure.
	(VisualStudio Setup Instructions): Remove filename
	extension from the list of supported configuration
	options so that the provided names match the names
	allowed by the configure script.
