Clicking "Close" on portlet means portlet is lost for good

This change is now done.

The solution was to add the "container" name to the query parameters for the
actions on a control.

The Close action then looks up the portlets in that container and removes the
entry that is specified in the request, based on name.

This allows for the same portlet to be used in different tabs/containers, but
will remove all portlets with the same name in the same container.

Also replaced the turbine/default.psml with the version produced by jetspeed
itself - as the old version seemed to cause a glitch with the close action.

Also amended the build.xml clean action to specifically delete *.java files in
the generated directories, rather than excluding the CVS files - this stops the
package.html files being deleted - they are not generated.
