Why do you need both:  
 
    <unique name="JETSPEED_USER_PROFILE_UNIQUE"> ...

and

    <index name="JETSPEED_USER_PROFILE_INDEX">

i believe if you just have:

    <unique name="JETSPEED_USER_PROFILE_UNIQUE">

its good enough, the unique index is created.
