When PhoenixTransactionContext.getTransactionalTable() or getTransactionalTableWriter() are
called, but the transaction has not yet been started, we should either:
 * throw an exception or
 * set the transaction on all OmidTransactionTable instances that have been opened before

See FIXME in FlappingTransactionIT.testExternalTxContext() for an example
