I also have that sometimes. It happens (I think) if a Perl interpreter
in Apache is restarted because of perlInterpMaxRequests is reached or
because one crashed.
If I immediately restart Apache after I do a fresh start, it always
restarts fine.
I do also a 'stop' and then after the service stopped, a 'start' that
works most of the time.
