Is it possible that you have 15-20 servers running? Try starting only one 
server with
% httpd -DONE_PROCESS -DNO_DETACH
If it now disappears after the first request, then you have the usual problem 
of persistent variables. See the perl.apache.org docs for more.

Besides please follow these http://perl.apache.org/bugs/ guidelines on how to 
report problems.
