You mean the filter wasn't loaded until second request? (You should try with a 
single server mode to figure out on which request it comes up, use

   httpd -DONE_PROCESS -DNO_DETACH

Next, you can enable tracing and run with env var MOD_PERL_TRACE=h, which will 
write to error_log all the info about handlers. Please see:
