there is a big difference between CORE::exit and Apache::exit, perlvar is
not refering to Apache::exit :)
but your handler works fine for me with 5.005_03 and 5.005_67 anyhow.

maybe custom_response will help you:

my $r = shift;

$r->custom_response(500, "Child $$\n");

die "whoops";
