Server: Apache/1.3.15-dev (Unix) mod_perl/1.24_02-dev (both HEAD in CVS)
perl 5.6.0 (old build for FreeBSD 4.0-RELEASE; see perl -V below)

% perl Makefile.PL DO_HTTPD=1 APACHE_SRC=../../httpd/apache-1.3/src \
  USE_APACI=1 EVERYTHING=1

Seems to function as expected.
