Thanks for your reply.
I'm going to a bit more testing myself before sharing the form and code.
I had some (as yet unpredictable) success seeing the form values after
saving the PDF with Acrobat Reader.
I may be able to make progress on my own.  If not, I'll come back here to
post the form, etc.
