On Mwiki you can build a "tree" of pages. For example,

Writer (with the index for that chapter)
Writer/Introduction
Writer/Introduction/Editing_simple_text
...
Writer/Styles
Writer/Styles/Paragraph
...

You can see something quite similar to the proposed TOC on the ES wiki
