Considerable progress.  Now only one of the twenty tests fails.

TestEntity refreshRemoved = new TestEntity("refresh removed");
em.persist(refreshRemoved);
em.flush();
em.remove(refreshRemoved);
em.flush();
em.refresh(refreshRemoved);

This should throw an IllegalArgumentException, but does not.
