Hi, this is not a bug - you have 

requestContext.launchDialog(dialog, null, // not launched from any
component


in your source, but ReturnEvent is delivered from a component. 

Get CommandButton component (from binding or ActionEvent) and put it as
third parameter to  launchDialog method.
