The chapter numbering is a general problem for aggregating Sinks, I have related issues with
the pdf plugin (MPDF-10). The problem is  that there is no notion of 'chapter' or 'part' in
the Sink API. I think at the very least we should add a chapter element to the DocumentModel
TOC but how this will integrate with the book code, no idea.

The links are working for me or am I missing something?
