CONNECTORS-1048 is "done", although I have some refinements I'd like to do still.

I'll look into revising the start.jar way of starting ManifoldCF next and converting those
to scripts.  I'm still thinking about relying entirely on combined.war, but I think we get
some testing benefit by having a combined execution model that runs all the independent web
applications.  So maybe we still will have both.
