This looks like a rather serious error.  It seems like that file has
somehow gone missing in your index.

On adding a given document, Lucene has flushed its buffered documents
and is attempting to merge segments together when it finds that one of
the segments that it thinks exists (_v3.cfs) and needs to open for
merging, can't actually be opened.

Were there any other exceptions before this one?  Is the error easily
reproduced?  If so, does it always hit the exception on that one file?

One thing to try is to call IndexWriter.setInfoStream(System.out)
which will cause the writer & deleter to print verbose details of
merges/deletions taking place.  That could give us a clue on the root
cause.
