Can we restrict this change to just be the alterations to CachedMatchesIterator?  And make
sure that we have a test for the new behaviour in TestIntervals directly, rather than relying
on highlighter tests to check the behaviour?  I think the changes to MultiTermIntervalsSource.visit()
need rethinking - wrapping things in a fake query seems like a hack just to support a particular
way of highlighting things, which feels wrong to me.
