Sivan Yogev updated LUCENE-4258:

1. Removed some assertions which were based on assumptions that stacked segments break
2. Added a mechanism to apply updates on already applied ones, now all tests pass
3. Did some house cleaning
What's left? Improve calculation of bytes used, make merge policy updates-aware, add the option
to collapse stacked segments for segments that cannot be merged, check performance...
