I agree with Hoss' view that mm should apply to all _top-level_ {{SHOULD}} clauses in the
query, especially since that's how it works in dismax.

The {{q.op}} and/or {{<defaultOperator>}} should definitely not get overridden when
mm is at least not specified.

In our situation we currently have to add an {{AND}} operator to fix queries that have a {{NOT}}
or {{-}} operator in them.

Does anyone have a patch to fix this behaviour?
