more wrapped-reader inconsistencies/bugs

                 Key: LUCENE-3812
             Project: Lucene - Java
          Issue Type: Bug
            Reporter: Robert Muir

I added a maybeWrap to testDuelingCodecs and ran it a while (like 50 iterations) before committing.

of course then i made an unrelated change and it randomly failed:
ant test -Dtestcase=TestDuelingCodecs -Dtestmethod=testEquals -Dtests.seed=27a287947a153c89:1e9e948a14bdc1ea:5c4e1854155cfa15

Some kind of wrapped-reader/codec is returning unindexed fields in its FieldsEnum
