As per discussion, this patch removes byte block pool forwarding address rewrites by always
allocating 4 bytes at the end of each slice.  newSlice has been replaced with newSliceByLevel
because we were always calling this with the first level size.  TestByteSlices passes. 

With this working, we will not need to implement byte block copy-on-write.  Instead, a posting-upto
per reader will be used.
