Removing a file from index does not remove all references to file.

If I walk though a list of files and remove them from 
the index, it appears as only half of the references are
really removed.  

I have included the java class I use to create and update
the index below.  If you have any questions, please let me
know.  I can also provide the intellij plugin (with 
source) that does the indexing if you want to use it
for debugging.

In the source below, if I index all the ANT manual API's, and
do a search for "Ant", I get 531 hits.  After doing the remove
operation where I remove all the ant manuals docs from the
index, I get around 260 hits.  On windows, I sometimes continue
to get 531 hits.
