We have the same problem while running 1.2.11 - it looks as though the renaming of the current
file to the .1 file fails as we end up missing the .1 file and that would make sense then
that the running file gets stomped with a new one.

Looking at the code I would have expected to get some other warning for the rename failure,
but we do not.
