That may be my problem! I had not filled in any values in the cookie
manager as I thought that was solely for hardcoding. So now I have:
Name: JSESSIONID
Value: <blank>
Domain: <blank>
Path: /
Secure: <ticked>
Expiration: 0
And I have 'Clear cookies each iteration' ticked...however I am still
getting a different session id :(
Do I need to put anything in value / domain?

OK. I've cut out everything relating to rewriting and just trying with a
blank (i.e. no hardcoded cookies) cookie manager.
However it does not seem to retain the cookie. Bear in mind I have 3
consecutive http samplers, the first one returns a cookie, the next one
obviously is not using the same cookie as it is returning a different
session id.
This did work when I had a single user, I added a cookie in to the manager
and it all worked. But in this instance I want to maintain a different
session for each thread.

That's a cookie - use the Cookie Manager.
