Okay, Gavin, here's what I came up with (in attached files).

These also include the ##param lines that define the parameters that users see in the Confluence
macro browser, at least in later Confluence versions. So they should be fairly future-proof.

I tested these in lots of different scenarios with missing & present parameters &
body, and all work well.

I also added identifying CSS names to the parts, so that if needed they can be styled in the
future (e.g. in the CSS that Tapestry's export process uses.
