The ValueWriter classes uses a simple HashMap for keeping track of which objects have already
been written to the stream and substituting indirection.  This can result in false positives
on the indirection test, depending on the object implementation of equals().   This needs
to be an IdentityHashMap to perform the indexing using the object identity.
