Subject [Bug 120484] original .doc files header & body content not in correct order when opened with writer

I tried to replicate this bug on Mac OS X (version 10.7.4).
When opening your document (Bonding1.doc) with OpenOffice 3, the header and
body content were indeed mixed. However, I copied your document in a .pages
file on Mac, fixing the header, footer, and just copying the rest of the
content.
 I then exported it to .doc. When opening the new .doc file with OpenOffice,
the format was actually correct and no content was mixed. I included snapshots
of what the document looks like on my computer. 

Was the Bonding1.doc created with OpenOffice or another application? It seems
to work fine when opening a .doc which was created through Mac Pages.
