I've experienced similar problem.  But I was using the Tomcat's servlet container's authorization

I was able to get around the 2nd issue.  

After loggin in, enter something in the 'Name' and 'Email' field in the profile page where
it displays the error message "You cannot set your login name...".  After you click save,
you should see your profile created and a better error message "You cannot set your login
name because your credentials are managed by the web container, not the wiki."
