Minor patch, 'fixing' 2 issues:

- presentation of the 'disclaimer'. The button placement kind of ruined the visual balance.
I've replace the button with a text link and updated the dialog with some enhancements;
- updated the reading of the config XML URL from the FlashVars in the HTML template. In the
original version, the var for the config file name also contained the URL, which was already
available in a different var. I've removed the URL from the name and updated the 'fetch' command
so it concatenates the URL and the name.
