Yeah, but the alternative is to use a method that doesn't work.  Even less 
appealing.

Exactly. There should be a Content-Disposition header that includes the filename 
of the attachment.

If you do that you’re going to create multiple revisions, one for each time you 
PUT the document. You may not want that.
