Hey Garren,


Whenever we add third party assets to Fauxton (and CouchDB as a whole), we
need to add the appropriate license and copyright information. Can you:

1) Update LICENSE with the appropriate license info for jquery.form.js
2) Update NOTICE with the appropriate copyright info
