So something like a CustomBufferedIndexOutput with a subclass CachedIndexOutput ? Once a buffer
is full flush it to the cache/disk.

Perhaps some trickiness in ensuring we handle calls like org.apache.blur.store.blockcache.BlockDirectory.fileLength(String)
