Maybe the easiest would be to use a rewrite:

RewriteCond %{HTTP_HOST} !^www\.
RewriteCond %{HTTP_HOST} ^([^.]+)\.domain\.com
RewriteRule ^/(.*)$  /home/%1/www/$1 [L]
