To be somewhat more thorough, or at least to allow for this kind of potential
typo, try

AddType text/html .shtml .shtm
AddOutputFilter INCLUDES .shtml .shtm

And just as critical, there must be a space before the closing '-->' as
well:

    <!--#include FILE="./include/topnav_home.inc"-->     BAD
    <!--#include FILE="./include/topnav_home.inc" -->    GOOD
